Dr. Rajendra Singh Bhati (MAN Hospital, Guna) decides why and how your details are used and is the Data Fiduciary under the Digital Personal Data Protection Act, 2023. This notice covers this appointment website only. Your treatment records are kept by the hospital, not here.

• Mobile number: to sign you in with a one-time code and to send SMS about your appointment.
• Name, date of birth, gender, address: to identify you at the desk and prepare your visit.
• Appointments: date, time slot, token number and status, to run the queue.
• Your consent: when you agreed and to which version of this notice.
• Sign-in records: each time you sign in, your network (IP) address, device type and the time are recorded, and kept for 365 days, to investigate misuse of an account. Your network address is also held for a day to stop guessing of one-time codes. Staff actions on the desk are logged.

We do not use your data for advertising, selling or profiling, and we collect nothing else about you from this website.

Visiting a psychiatrist is sensitive. We treat the fact that you have an appointment as confidential, as the Mental Healthcare Act, 2017 expects. The SMS we send names the clinic; if others use your phone, you can cancel by phone instead of by SMS and tell the desk.

Supabase (database and sign-in) and Vercel (website hosting), both used in India (Mumbai), and MSG91 (one-time codes and SMS). They process data only to provide those services.

• Appointment history: 3 years after the visit, then deleted.
• Your account: deleted after 3 years with no visit and no sign-in, or earlier when you ask.
• Copies of SMS sent: 30 days.
• Sign-in records and records of who accessed or changed data: 365 days, as the law requires logs to be kept. These remain for that period even if you erase your account.

You may see a copy of your data, correct it, erase it, withdraw your consent, and name someone to exercise these rights for you if you cannot.

• Signed in: Profile has Download my data, edit, and Delete my account.
• Otherwise: ask the desk or write to the Grievance Officer below.

If you are under 18 (or cannot manage your own affairs), a parent or lawful guardian must book and give consent for you.

• अपॉइंटमेंट: तारीख़, समय, टोकन नंबर और स्थिति, ताकि कतार चल सके।
• आपकी सहमति: कब और इस सूचना के किस संस्करण पर आपने सहमति दी।
• साइन-इन रिकॉर्ड: हर बार साइन-इन पर आपका नेटवर्क (IP) पता, डिवाइस का प्रकार और समय दर्ज होता है, जो खाते के दुरुपयोग की जाँच के लिए 365 दिन रखा जाता है। OTP का अंदाज़ा लगाने से रोकने के लिए नेटवर्क पता एक दिन भी रखा जाता है; डेस्क पर स्टाफ़ की कार्रवाइयों का लॉग रखा जाता है।

मनोचिकित्सक से मिलना संवेदनशील बात है। मानसिक स्वास्थ्य देखभाल अधिनियम, 2017 के अनुसार हम आपकी अपॉइंटमेंट को गोपनीय मानते हैं। SMS में क्लिनिक का नाम होता है; अगर आपका फ़ोन कोई और भी इस्तेमाल करता है तो SMS की जगह फ़ोन पर रद्द करें और डेस्क को बताएँ।

Supabase (डेटाबेस और साइन-इन), Vercel (वेबसाइट होस्टिंग), दोनों भारत (मुंबई) में, और MSG91 (OTP और SMS)। ये सिर्फ़ ये सेवाएँ देने के लिए डेटा संभालते हैं।

• अपॉइंटमेंट का इतिहास: विज़िट के 3 साल बाद हटा दिया जाता है।
• आपका खाता: 3 साल तक कोई विज़िट या साइन-इन न होने पर हटा दिया जाता है, या आपके कहने पर पहले।
• भेजे गए SMS की प्रति: 30 दिन।
• साइन-इन रिकॉर्ड और किसने डेटा देखा या बदला इसका रिकॉर्ड: 365 दिन, जैसा कानून में ज़रूरी है। खाता हटाने के बाद भी ये इस अवधि तक रहते हैं।

आप अपने डेटा की प्रति देख सकते हैं, सुधार करा सकते हैं, मिटवा सकते हैं, सहमति वापस ले सकते हैं, और अपनी ओर से किसी को नामित कर सकते हैं।

• साइन-इन करके: प्रोफ़ाइल में “मेरा डेटा डाउनलोड करें”, बदलाव और “मेरा खाता हटाएँ” है।
• अन्यथा डेस्क से कहें या नीचे दिए शिकायत अधिकारी को लिखें।

18 वर्ष से कम उम्र के लिए माता-पिता या कानूनी अभिभावक को बुकिंग और सहमति देनी होगी।
